Thank you for registering your player to play baseball at BHLL in Spring 2020!

BEFORE YOU START: You should expect to download, fill out, then upload the normal Medical Release form. You’ll also need to upload the required Residency Verification docs (please have a Driver’s License, any government doc such as first page of tax return or property tax bill, PG&E or Cable/satellite/internet bill.) You must show us one from each category, as described on the Proof of Residence page here:

Note you can and should delete (black-out) any private information from these – all we need to see is your name and address on the documents.
